Understanding Herniated Discs & Bulging Discs
A decent analogy for understanding herniated discs is a jelly donut. If you put enough pressure on a jelly donut (all at once or gradually over time), its outer doughy layer will tear and the inner jelly can leak out. In a similar way, abnormal pressure on a spinal disc—soft structures located between vertebral bones—can cause its tough outer layer to rupture and allow its inner gel-like substance to leak out into the surrounding area. When this happens, nearby structures, such as nerve roots, can be compressed and irritated.
Abnormal pressure on a disc can come from things like excessive sitting, heavy lifting, poor lifting mechanics, or even an auto accident.
A bulging disc is slightly different. To use the donut analogy, this would be akin to squeezing a jelly donut so the jelly bulges and moves out of place, but doesn't actually leak through the outer layer. In this way, bulging discs can still protrude against nearby structures.
Common Herniated Disc Symptoms
Since spinal discs themselves are poorly innervated, they may not cause any pain directly. However, symptoms can occur if nearby nerve roots are impinged. This can cause numbness, tingling, and weakness in an arm (if the disc herniation is in the upper spine) or a leg (if the disc herniation is in the lower spine).
Additional symptoms may include tenderness and muscle spasms in the back, back stiffness, and limb pain that centralizes (moves closer to the spine) with trunk flexion or peripheralizes (moves further down the limb) with trunk extension.
